Questionnaire

A data gathering method.

A questionnaire is a set of questions that seek some specific information. Questions can be open, where the answer could be anything. For example "What is your favourite food", or they can be closed - where the answer is very specific, for example "How many children do you have?"

A questionnaire seeks to find out information about a topic so that it can be understood and analysed in some way. For example an opinion poll questionnaire reveals what people think about a topic. Or if you are designing a new system, part of the "Systems Life Cycle" process is to gather information from users and customers. One way to do this is to run a questionnaire.

Questionnaires can be paper-based or electronic, but either way, the data normally ends up in a spreadsheet or database that can summarise the results.
